Factors associated with unplanned reinterventions and their relation to early mortality after pediatric cardiac surgery

ObjectiveUnplanned reintervention (uRE) is used as an indicator of patient morbidity and quality care in pediatric cardiac surgery. We investigated associated factors early mortality after uREs.MethodsMorbidity data were prospectively collected 5 UK centers between 2015 2017; uRE included surgical cardiac, interventional transcatheter permanent pacemaker, diaphragm plication procedures. Mortality (30-day 6-month) uRE/no-uRE patients was reported before matching. Predicted 30-day calculated using the Partial Risk Adjustment Surgery score.ResultsA total 3090 procedures (2861 patients) (median age, 228 days). There 146 uREs, resulting rate 4.7%. score, 6-month no-uRE groups 2.4% versus 1.3%, 8.9% 1%, 17.1% 2.4%, respectively. After matching, at 6 months remained higher compared with (12.2% vs 1.4%; P = .02; 74 pairs). In group, 21 out 25 deaths occurred when least 1 additional postoperative complication present. multivariable analysis, neonatal age (P .002), low weight .009), univentricular heart < .001), arterial shunt .001) increased risk uRE, but score not (only univariable analysis).ConclusionsuREs are a relatively frequent surgery some characteristics, score.
